NCLEX Prioritization Made Simple: ABCs, Maslow's, and Safety

Somewhere in your NCLEX-RN there is a question asking which patient to see first, and every option looks urgent. That question is less about knowledge and more about knowing how to rank what you already know. This guide gives you three lenses to sort it out: the ABCs, Maslow's hierarchy, and a safety mindset that the exam rewards.

The Scenario: Four Patients, One You

Imagine the shift you are given on a prioritization question. One patient is short of breath and has stridor. One is fasting and upset because breakfast is late. One had surgery yesterday and is still sleepy. One is an older adult who tried to get out of bed and nearly fell. Every one of them 'needs you.' The exam is not asking who is nice to help - it is asking who will be harmed if you do nothing.

Your job is to build a mental filter that runs in about ten seconds. Run the ABCs first, revisit with the safety mindset, and when two options are still neck and neck, let Maslow break the tie. You will do all three below, on real-style scenarios, so the reasoning is automatic by test day.

The ABC Framework: Start Where Breathing Stops

The ABCs - airway, breathing, circulation - are your first pass on every prioritization question, and they map directly to how harm actually happens. A blocked airway or a failing airway will kill faster than pain, hunger, or a low lab value. So when you scan the options, ask: is anyone's airway threatened, is anyone not breathing well, or is anyone's circulation unstable? That patient goes first, full stop.

Circulation matters more than almost any single number on the screen, and airway and breathing outrank everything else. If a patient cannot move air, nothing else you do that minute matters. Use the ABCs to establish an absolute first priority, then move on to the subtler work of ranking everything that survives the first pass.

Once the ABC check is done and nothing is immediately life-threatening, you have dropped the noise and are left with a short list of real candidates. That is exactly where the safety mindset and Maslow's hierarchy earn their keep.

The Safety Mindset: Why 'First, Do No Harm' Wins Points

Before you memorize a single framework, lock in a safety mindset, because it is the whole point of the exam. Around 1 in every 10 patients is harmed in health care and more than 3 million deaths occur annually due to unsafe care. More importantly for you, above 50% of harm (1 in every 20 patients) is preventable; half of this harm is attributed to medications. Most of what the NCLEX asks you to prevent lands squarely in that preventable bucket.

The safety framing is not fear-mongering - it is the exam's grading rubric in disguise. Decades ago, the Institute of Medicine's To Err Is Human estimated that 44,000-98,000 Americans die each year due to preventable harm, and reviews since then still find that 10%-12% of hospitalized patients experience adverse events, with approximately half of these events considered preventable.

A safety mindset also means using the patient as a partner, which is far more powerful than it sounds. Patient engagement, if done well, can reduce the burden of harm by up to 15%. That is a strong argument for prioritizing the patient who can still tell you what is wrong and what changed.

And harm is not abstract - it is concrete and common. Falls are one of the most frequent and dangerous complications in hospital and long-term care. So part of thinking like a safe nurse is asking: who is about to fall, and who is about to get a medication error?

Maslow's Hierarchy: The Tie-Breaker

Maslow's hierarchy is the exam's way of settling questions where the ABCs alone do not decide. It ranks needs from the most basic and urgent up to the highest: physiological needs (air, food, water, elimination, sleep), then safety and security, then love and belonging, then esteem, and finally self-actualization.

The rule you actually need: lower needs win over higher ones. A physiological need beats a safety need, and a safety need beats a comfort or emotional need. So a patient who cannot breathe or has unstable vital signs beats the patient who is lonely or is anxious about a procedure.

Maslow shines in the gray zone. When two patients both pass the ABC scan, compare where they sit on the hierarchy - the patient nearer the bottom (breathing, circulation, pain, fall risk) gets seen before the patient whose need is social or emotional. Use the ABCs for the emergency, then let Maslow rank the survivors.

Worked Examples: Watch the Reasoning, Not the Answer

Example 1 - The stridor question. Four patients: one with stridor and audible wheezing, one with a slow oozing surgical drain, one fasting and hungry, one worried about a biopsy. The ABC pass is immediate - airway is threatened in the stridor patient, so that is the first priority. The hungry patient ranks lowest on Maslow (a comfort need beats nothing, but not the physical ones). Even though the drain is a real circulation concern, nothing outranks a compromised airway.

Example 2 - The confused fall risk. An older adult with delirium keeps climbing out of bed, and another patient is asking for pain medication after minor discomfort. This is where safety reorders your thinking. Most falls occur in elderly patients, especially those who are experiencing delirium, are prescribed psychoactive medications such as benzodiazepines, or have baseline difficulties with strength, mobility, or balance. The math behind it is sobering: falls occur at a rate of 3-5 per 1000 bed-days, and the Agency for Healthcare Research and Quality estimates that 700,000 to 1 million hospitalized patients fall each year.

Example 3 - Injury makes it a hard priority. If you are weighing a fall-risk patient against a comfortable one, let the consequences drive you. More than one-third of in-hospital falls result in injury, including serious injuries such as fractures and head trauma. Fall prevention is your job here - the bed alarm, the lower bed, the call light within reach - before you go answer the less urgent bell. That is prioritizing the patient who could be permanently harmed over the one who is merely annoyed.

Common Traps That Cost You Points

Trap 1 - Treating a near miss like it never happened. A near miss is an unsafe situation that is indistinguishable from a preventable adverse event except for the outcome. The only reason the patient was not harmed was luck or early detection. On the exam, a near miss is not a pass - it is a red flag that the patient still needs protection and follow-through.

Trap 2 - Forgetting that an omission counts as an error. An error is any act of commission (doing something wrong) or omission (failing to do the right thing) that exposes patients to a potentially hazardous situation. Failing to raise the side rail or skipping the fall-risk screening is an error just like giving the wrong drug is. Safety questions often test the thing you failed to do, not the thing you did.

Trap 3 - Believing the diagnosis on the screen is the whole story. Diagnostic errors occur in 5-20% of physician-patient encounters, so the written diagnosis is a starting point, not gospel. Run your own ABC check and your own safety screen instead of going on autopilot from the label.

Trap 4 - Treating falls as routine. Death or serious injury resulting from a fall while being cared for in a health care facility is considered a never event. When a fall is even a possibility, that patient jumps your priority list - and so does the prevention work that keeps falls from happening at all.

Your Prioritization Checklist

Run this list on every prioritization question so the process, not the guessing, does the work:

  • Airway and breathing first - compromised airway or breathing beats everything.
  • Circulation next - unstable circulation outranks comfort and convenience.
  • Apply Maslow to break ties - lower, more basic needs beat higher social or emotional ones.
  • Ask the safety question - is anyone at risk of a medication error, a fall, or a missed sign of deterioration?
  • Check what you would omit - errors include both wrong actions and things you fail to do.
  • Don't trust the label blindly - screen the patient yourself rather than the diagnosis on the screen.
